Outreach Nurse – Community Health (Auckland)

About the role

The Outreach Nurse role supports high‑risk, marginalized communities in central Auckland, aiming to improve health outcomes and engagement with services. You will work within the Mission’s values‑led environment, receiving mentorship and on‑the‑job development.

Key responsibilities

• Utilise community networks to build relationships with clients and facilitate engagement with General Practice teams and other primary health care providers.
• Partner with healthcare providers, social agencies, community groups and employers to ensure timely access to services and referrals.
• Empower clients to manage their own health and make informed decisions.
• Promote appropriate referral to specialised services and education classes through clear pathways.
• Make nursing judgments based on current knowledge, research and reflective practice.

Required profile

• 3‑4 years recent experience in a primary care setting, preferably working with Māori and/or Pacific Island communities and at‑risk populations.
• Current practising certificate as a Registered Nurse.
• Demonstrated commitment to Te Tiriti o Waitangi and the Mission’s values of Manaakitanga, Atawhai, Rangapū, Mana Tika and Mana Ōrite.
• Experience working with alcohol, drug and mental health consumers.
• Understanding of Trauma‑Informed Care.
• Full clean NZ driver’s licence.
