Practice Pharmacist – CPCT Team (Otara)

About the role

The CPCT Practice Pharmacist will join the Otara Family and Christian Health Centre as the medicines expert for the practice. Working directly with patients and whānau, you will provide personalised pharmacotherapy advice, deliver education sessions and help reduce medicine‑related harms.

Key responsibilities

• Manage, optimise and reconcile medicines for patients.
• Conduct clinical evaluations and implement best‑practice medication protocols.
• Lead medication audits, quality‑improvement projects and ensure equitable access to medicines.
• Develop and oversee standards, policies and procedures for safe and effective medicine use.
• Participate actively in an interdisciplinary team (IDT) to enhance team functioning.
• Build and strengthen partnerships with other health and wellbeing providers.

Required profile

• Current APC registration with no restrictions.
• At least 2 years experience as a registered pharmacist in primary or secondary care.
• Post‑graduate study in clinical pharmacy or pharmacist prescribing (completed or in progress).
• Strong understanding of health inequities in Aotearoa New Zealand and application of Te Tiriti o Waitangi.
• Cultural competency with knowledge of at‑risk communities’ languages and cultures.
• Experience in collaborative, multi‑disciplinary health teams (advantageous).
• Experience supporting long‑term condition management through medication initiation and continuation (highly regarded).
