TOC Clinical Pharmacist

About the role

The TOC Clinical Pharmacist at Mease Dunedin Hospital will help reduce readmissions by providing post‑discharge medication support and patient education. This full‑time, onsite position focuses on medication safety and continuity of care for recently discharged patients.

Key responsibilities

• Conduct patient discharge follow‑up to ensure medication adherence.
• Promote medication safety and address drug‑related problems.
• Educate patients on medication regimens and disease states.
• Facilitate provider follow‑up and continuity of care after hospital discharge.
• Collaborate with the interdisciplinary team to achieve readmission reduction goals.

Required profile

• Active Pharmacist license in the State of Florida.
• Bachelor’s degree required; PharmD preferred.
• Preferred certifications: ASHP (Pharmacist).
• Preferred experience: PGY‑1 or PGY‑2 residency.
