About the role

Care on Call (NZ) Limited is seeking a Registered Nurse to join its community clinical team in Wellington. The role involves delivering high‑quality, person‑centred care to clients recovering from or living with injury or illness, primarily in home‑based settings.

Key responsibilities

• Conduct clinical assessments and develop service plans for long‑term and complex clients.
• Provide wound care, vaccinations, and single‑discipline assessments.
• Respond to acute events and complete InterRAI assessments.
• Participate in case management, funding coordination, and multidisciplinary reviews.
• Maintain accurate documentation and uphold professional standards.

Required profile

• Registered Nurse with a current Annual Practising Certificate (APC) in New Zealand.
• Minimum three years of nursing experience across home‑based, facility, or DHB environments.
• Strong attention to detail and ability to work autonomously.
• Commitment to outcomes‑focused, person‑centred care.
