About the role
We are looking for a registered mental health professional to join a South Auckland general practice as a Health Improvement Practitioner. The role sits within the innovative Te Tumu Waiora programme and focuses on brief, evidence‑based interventions that empower patients to achieve their own health and wellbeing goals.
Key responsibilities
- Deliver brief psychological interventions, especially Focused Acceptance and Commitment Therapy (FACT), to individual patients and groups.
- Provide consultation, education and advice to the general practice team, including GPs and nurses.
- Develop and implement intervention pathways tailored to the clinic’s population health needs.
- Collaborate with a network of Fresh Minds Health Improvement Practitioners and health coaches.
Required profile
- Fully registered mental health professional (psychologist, psychotherapist, mental health nurse, social worker, occupational therapist, or DAPA).
- Commitment to culturally responsive care for Māori, Pasifika, and youth communities.
- Ability to work within a multidisciplinary primary‑care team.
Required skills
- Experience delivering brief talking therapies.
- Knowledge of Acceptance and Commitment Therapy.
- Strong communication and collaborative skills.
