About the role
We are seeking a Clinical Nurse Specialist with a focus on diabetes to join our team in Palmerston North Central. This part‑time, on‑site position offers the chance to use your specialist knowledge to improve health outcomes for individuals and whānau living with diabetes. You will play a pivotal role in delivering high‑quality diabetes care and supporting community health initiatives.
Key responsibilities
- Provide specialist diabetes care and education to patients and families.
- Develop and implement individualized care plans aimed at improving glycaemic control.
- Monitor patient progress and adjust care plans as needed.
- Collaborate with multidisciplinary teams to promote community health initiatives.
- Empower people with diabetes to manage their condition effectively.
- Provide training to staff on diabetes best practices.
- Participate in quality improvement projects related to diabetes services.
