About the role
Join Waipuna Hospice as a Registered Nurse delivering community palliative care in Te Puna, supporting patients across Tauranga and the Western Bay of Plenty. You will work semi‑autonomously to provide compassionate, holistic care that enhances quality of life for people living with life‑limiting illness and their whānau.
Key responsibilities
- Conduct skilled holistic assessments for patients with complex, evolving care needs.
- Coordinate proactive palliative care management, focusing on symptom control and comfort.
- Facilitate seamless transitions between community and inpatient settings.
- Participate in triage, provide advice, and deliver flexible service as required.
- Lead serious illness conversations and advance care planning.
- Collaborate with Māori health providers and community partners.
- Contribute to quality improvement, service development, and clinical leadership.
Required profile
- Registered Nurse qualification.
- Experience in community or palliative care settings.
- Ability to work independently and as part of a multidisciplinary team.
- Strong communication skills with patients, whānau, and health professionals.
